Requirements:
- Application for notarial services form (available at the Consular Section of the Embassy);
- NBI fingerprint card form (also available upon request at the Consular Section);
- 2x2 white background picture;
- copy of passport or any valid ID
Procedure:
- Submit the required documents to the Consular Section (counter no. 5);
- pay the required fee at the cashier; have your finger prints taken by a Consular Assistant;
- with your invoice claim your NBI fingerprint card form within the same day
NOTE: The accomplished NBI fingerprint card form may be submitted to the NBI through a representative to complete the process of the issuance of your NBI clearance.
